This durable Vermont power of attorney form allows an individual to designate a trusted person (an “agent”) to manage their financial matters or make medical decisions on their behalf. The powers granted to the chosen agent can be specific or extensive, depending on the wishes of the person making the transfer (the “principal”).
The Principal must sign the POA document in front of a public notary.
