Required Disclosure Forms

    1. Lead-Based Paint Disclosure and EPA Pamphlet– Required for homes built before 1978.
    2. Allergen Hazards Disclosure (NYC only) – Landlords must confirm an annual inspection for mold, mice, rats, and cockroaches with the Local 55 brochure. [1]
    3. Bedbug Disclosure (NYC only) – Landlords must disclose the unit’s bedbug history from the past year. [2]
    4. Copy of signed lease (Rent-stabilized tenants only) – This must be provided within 30 days of the start date of the tenancy. [3]
    5. Flood History Disclosure – Tenants must know past floods, risks, and insurance details. [4]
    6. Good Cause Eviction Notice – The lease must state if the tenancy falls under this law. [5]
    7. Security Deposit Receipt – If collected, Landlords must disclose where it is held.
    8. Sprinkler Disclosure – The lease must boldly state whether a sprinkler system is installed.
    9. Stove Knob Covers (NYC only, if applicable) – If tenants have children under the age of six (6) years old, the Landlord must provide stove knob covers.
