Required Disclosure Forms

Landlord’s Disclosure (§ 521-43(a)↗) – The landlord must share their contact details and names of anyone allowed to access the property.

  • General Excise Tax Number: This number must be provided so tenants can apply for a low-income tax credit. It can be found by searching the landlord’s name on the “Search Tax Licenses” webpage.

Move-In Condition Report (§ 521-42(6)↗)- The landlord must give the tenant a report on the property’s current condition before signing the lease.

Lead-Based Paint Disclosure (hud.gov↗)- If the property was built before 1978, the landlord must provide information about lead-based paint hazards and an EPA safety pamphlet.
